The aim of breast liposuction among men is a simple procedure, improving a patient’s physical appearance by removing as much fat accumulation as possible, while avoiding at the same time skin and muscles damages. This is probably the most common liposuction treated area of the male body (along with the abdomen, chin, cheeks and lips).
Normal male breasts consist of both fat and fibrous glandular tissue. Male breast tissue is located just below the nipple and is represented as a localized “lump” which is stiffer than the surrounding fat tissue. Removal of fat tissue by liposuction is а significantly easier procedure than liposuction of glandular tissue. However, distinguishing glandular from fat tissue might sometimes be difficult and in this case mammography as a method can be used.

What is pseudogynecomastia
Pseudogynecomastia is an enlargement of the male breast caused by an excessive amount of fat tissue, with a normal amount of glandular tissue at the same time. In most cases, the reason for excessively large male breasts is the excessive fat amount. Mild pseudogynecomastia occurs in most men with aging. Excessively large breasts are a typical problem in younger men who are obese.
What is real gynecomastia
Real gynecomastia is an enlargement of the male breasts caused by overgrowth of glandular tissue. Real excessive glandular tissue in the breast is not usually common among men.
There are a number of reasons for gynecomastia to occur – alcoholism, insufficient production of testosterone from the testicles, the use of certain drugs, etc. Bilateral gynecomastia may also be caused by AIDS, hemodialysis treated renal failure, and some forms of cancer (e.g. testicular cancer). If a man has a single enlarged breast, then a check of possible breast tumor should be performed. Any significant asymmetry of the male breast, especially if it has a recent onset and asymmetric growth, is an indication for mammography to be performed.
Influence of some drugs
Drugs ,which can cause gynecomastia, include: amiloride (Moduretic), amiodarone (Cordarone), amphetamines, anabolic steroids, antiandrogens (cyproterone), anticancer (cytotoxic) drugs, androgens, busulfan (Mileran), capitenil, captopril (Clomid), diazepam (Valium), diethylpropion (Tenuat), digitalis, domperidone, estrogens, isoniazid, ketoconazole (Nizoral), marijuana, methyldopa, metoclopramide, nifedipine (Procardia), nitrosoreno, phenothyline, penicillin phenylene, phenylene, phenylene, phenylene, spironolactone (Aldactone), tricyclic antidepressants, vincristine.
Common temporary gynecomastia
This is a common condition among newborns (over 75%) and is caused by maternal estrogen hormones which pass into baby’s blood through the placenta during pregnancy. Throughout puberty, 60% of boys experience temporary breast augmentation, usually regressink after that within a few months.
Liposuction for enlarged male breasts
Liposuction is most effective in cases of pseudogynecomastia. In real gynecomastia, the breast tissue is dense and fibrous and therefore difficult to be both penetrated and removed with the use of a liposuction cannula. Microcannulas are effective in removing breast tissue as well as fat tissue from normally developed male breasts. If the glandular tissue is extremely dense, this may require surgical removal through a scalpel and a subsequent closure of the breast through sutures.
How liposuction of male breasts is conducted
Male breast liposuction is a well-known procedure, but the technique and the instruments for the operation are not standardized. Some surgeons prefer to perform liposuction entirely with the application of cannulas, while others prefer surgical excision of the excess tissue. Liposuction through local anesthesia
Tumescent liposuction of male breasts can be entirely undertaken through local anesthesia. After carefully injecting the local anesthesia into selected skin areas above the breast, multiple 1.5 mm incisions are made. A large amount of local anesthesia is then injected into. This reduces the resistance of the glandular tissue аagainst penetration. The smaller the diameter of the cannula, the easier it can penetrate dense tissues.
Postoperative care
After completing liposuction procedure, the incisions remain open (i.e. sutures are not applied) in order to maximize the drainage of residual blood and anesthetic solution. This drainage is absorbed by special tampons, which are placed with the use of spandex compression garment or a bandage. To maximize the compression, a special system of compression bandages and Velcro straps is applied, which ensures pressure control and regulation. To prevent bleeding and hematoma formation, the compression neck bandage is used within the first 24 hours.
What results can be expected after the procedure
Although perfect results should not be expected, a significant improvement in most cases can be anticipated. An expectation of an average 50% improvement is reasonable. Unfortunately, with some patients there is no likelihood of improvement. For example, in the case of a thin man with a minimum amount of excess breast fat, the improvement can hardly achieve more than 30%.
